Sudarshan Chakra is a devotee, sometimes He appears in a devotee form like when Sudarshan Chakra appeared as one of the 12 Alvars.

I was looking if Sudarshan Chakra appeared in Gaura lila, we know He appeared when Lord   Caitanya invoked Him 
to kill Madhai.
Lord Nityananda Prabhu prayed to Lord Caitanya: 
kono janme thake jadi amara sukrta saba dilun madhaire sunoho niscita more joto aparadha kichu daya nai maya charo krpa koro tomara madhai
O Lord! Please listen! If I have done any good deeds in this or any other life, I am giving it all to Madhai. I relieve him of all responsibility for his offenses to me. Please don’t play tricks, but bestow your mercy on Madhai. He is yours. (Cbh 2.13.219-20)
Gaura Ganodessa Dipika has not mentioned Sudarshan Chakra appearing personified in Gaura lila but that is not a complete list of Lord Caitanyas associates 
Sudarshan Chakra also appeared in Krsnas lilas when He killed Sisupala and other demons.
Also Sudarshan Chakra was used by Krsna invisibly during Krsnas ,Govardhana lila, and Sudarshan Chakra also helped Krsna burn up
the floods from the Samvartaka clouds send by Indra. At Govardhana at Govardhana city by Manasa Ganga there is Chakra Tirtha to honor Sudarshan Cakra, very close to the Sri Harideva temple.
There is several Chakra Tirthas in India related to Sudarshan Chakra, such as Chakra Tirtha in Dvaraka, where the ocean meets the Godavari river, the glories of this place is described in  Garga Samhita.  Sudarshan Chakra appears as the Dvaraka silas there.
They are sometimes called Gomati Salagram Silas.
There is Chakra Tirtha in Nepal where Salagrama Silas are at the Gandaki river there are many Sudarshan Salagrama silas as well.
The Brahma Purana states that wherever shalagrama-shila and Dwaraka sila sit  together, there certainly mukti (salvation) also resides. In the conversation  between Brahma and Narada in the Skanda Purana Brahma says, "O Munishvara!  Wherever Dwaraka sila sits in front of the shalagrama-shila every class of  opulence goes on increasing unlimitedly." In another place it says that one who  daily worships dvaraka-shila along with twelve shalagrama-shila will be honored  even in Vaikuntha-dhama.
The Varaha Purana says that whoever touches a shila with the mark of a  Vishnu-chakra will become free of all sins. The Garuda Purana declares that  simply the darshana of Sudarshana and other Dwaraka sila fulfils all desires.  The Skanda Purana declares that without a doubt if a very sinful man worships a  Dwaraka sila with devotion, or even without devotion, he becomes free from all  sinful reactions. According to the Dwaraka Mahatmya, a shila marked with chakras  coming from Dvaraka is also called chakra-tirtha. Even if this shila is  worshipped by a sinful person from a degraded country, the worships still  becomes liberated.
There is also Chakra Tirtha in Jagannath Puri.
In Krsnas pastimes Sudarshan also appeared personified when Krsna was about to leave Mother Earth and return to Goloka Vrindavana.
Śrīmad Bhāgavatam11.30.42
taḿ tatra tigma-dyubhir āyudhair vṛtaḿ
sneha-plutātmā nipapāta pādayo
SYNONYMS
tam — Him; tatra — there; tigma — brilliant; dyubhiḥ — the effulgence of which; āyudhaiḥ — by His weapons; vṛtam — surrounded; hi — indeed; aśvattha — of the ; mūle — at the base; kṛta-ketanam — resting; patim — his master; sneha — with affection; pluta — overwhelmed; ātmā — his heart; nipapātahe fell down; pādayoḥ — at His feet; rathāt — from the chariot; avaplutya — rushing down; sa-bāṣpa — filled with tears; locanaḥ — his eyes.
TRANSLATION
Upon seeing Lord Kṛṣṇa resting at the foot of a banyan tree, surrounded by His shining weapons, Dāruka could not control the affection he felt in his heart. His eyes filled with tears as he rushed down from the chariot and fell at the Lord's feet.
SB 11.30.44: [Śukadeva Gosvāmī continued:] O foremost of kings, while the chariot driver was still speaking, before his very eyes the Lord's chariot rose up into the sky along with its horses and its flag, which was marked with the emblem of Garuḍa.
SB 11.30.45: All the divine weapons of Viṣṇu rose up and followed the chariot. The Lord, Janārdana, then spoke to His chariot driver, who was most astonished to see all this.
Srimad Bhagavatam offers prayers to Sudarshan Chakra and Lord Visnus weapons in the 6th Canto:
cakraḿ yugāntānala-tigma-nemi
SYNONYMS
cakram — the disc of the Lord; yuga-anta — at the end of the millennium; anala — like the fire of devastation; tigma-nemi — with a sharp rim; bhramat — wandering; samantāt — on all sides; bhagavat-prayuktam — being engaged by the Lord; dandagdhi dandagdhi — please burn completely, please burn completely; ari-sainyam — the army of our enemies; āśu — immediately; kakṣam — dry grass; yathā — like; vāta-sakhaḥ — the friend of the wind; hutāśaḥ — blazing fire.
TRANSLATION
Set into motion by the Supreme Personality of Godhead and wandering in all the four , the disc of the Supreme Lord has sharp edges as destructive as the fire of devastation at the end of the millennium. As a blazing fire burns dry grass to ashes with the assistance of the breeze, may that Sudarśana cakra burn our enemies to ashes.
SB 6.8.27-28: May the glorification of the transcendental name, form, qualities and paraphernalia of the Supreme Personality of Godhead protect us from the influence of bad planets, meteors, envious human beings, serpents, scorpions, and animals like tigers and wolves. May it protect us from ghosts and the material elements like earth, water, fire and air, and may it also protect us from lightning and our past sins. We are always afraid of these hindrances to our auspicious life. Therefore, may they all be completely destroyed by the chanting of the Hare Kṛṣṇa mahā-mantra.
SB 6.8.29: Lord Garuḍa, the carrier of Lord Viṣṇu, is the most worshipable lord, for he is as powerful as the Supreme Lord Himself. He is the personified Vedas and is worshiped by selected verses. May he protect us from all dangerous conditions, and may Lord Viṣvaksena, the Personality of Godhead, also protect us from all dangers by His holy names.
SB 6.8.30: May the Supreme Personality of Godhead's holy names, His transcendental forms, His carriers and all the weapons decorating Him as personal associates protect our intelligence, senses, mind and life air from all dangers.
Then of course there is the prayers offered by Ambarish Maharaja to Sudarshan Chakra, he was a king in Mathura and Durvasa Muni made offences to Him, and Sudarshan Chakra chased Durvasa Muni,it is explained in SB 9th Canto .

SB 9.5.1: Śukadeva Gosvāmī said: When thus advised by Lord Viṣṇu, Durvāsā Muni, who was very much harassed by the Sudarśana cakra, immediately approached Mahārāja Ambarīṣa. Being very much aggrieved, the muni fell down and clasped the King's lotus feet.

SB 9.5.2: When Durvāsā touched his lotus feet, Mahārāja Ambarīṣa was very much ashamed, and when he saw Durvāsā attempting to offer , because of mercy he was aggrieved even more. Thus he immediately began offering prayers to the great weapon of the Supreme Personality of Godhead.

SB 9.5.3: Mahārāja Ambarīṣa said: O Sudarśana cakra, you are fire, you are the most powerful sun, and you are the moon, the master of all . You are water, earth and sky, you are the air, you are the five sense objects [sound, touch, form, taste and smell], and you are the senses also.

SB 9.5.4: O most favorite of Acyuta, the Supreme Personality of Godhead, you have thousands of spokes. O master of the material world, destroyer of all weapons, original vision of the Personality of Godhead, I offer my respectful obeisances unto you. Kindly give shelter and be auspicious to this brāhmaṇa.

SB 9.5.5: O Sudarśana wheel, you are religion, you are truth, you are encouraging statements, you are sacrifice, and you are the enjoyer of the fruits of sacrifice. You are the maintainer of the entire universe, and you are the supreme transcendental prowess in the hands of the Supreme Personality of Godhead. You are the original vision of the Lord, and therefore you are known as Sudarśana. Everything has been created by your activities, and therefore you are all-pervading.

SB 9.5.6: O Sudarśana, you have a very auspicious hub, and therefore you are the upholder of all religion. You are just like an inauspicious comet for the irreligious demons. Indeed, you are the maintainer of the three worlds, you are full of transcendental effulgence, you are as quick as the mind, and you are able to work wonders. I can simply utter the word namaḥ, offering all obeisances unto you.

SB 9.5.7: O master of speech, by your effulgence, full of religious principles, the darkness of the world is dissipated, and the knowledge of learned persons or great souls is manifested. Indeed, no one can surpass your effulgence, for all things, manifested and unmanifested, gross and subtle, superior and inferior, are but various forms of you that are manifested by your effulgence.

SB 9.5.8: O indefatigable one, when you are sent by the Supreme Personality of Godhead to enter among the soldiers of the Daityas and the Dānavas, you stay on the and unendingly separate their arms, bellies, thighs, legs and heads.

SB 9.5.9: O protector of the universe, you are engaged by the Supreme Personality of Godhead as His all-powerful weapon in killing the envious enemies. For the benefit of our entire dynasty, kindly favor this poor brāhmaṇa. This will certainly be a favor for all of us.

SB 9.5.10: If our family has given charity to the proper persons, if we have performed ritualistic ceremonies and sacrifices, if we have properly carried out our occupational duties, and if we have been guided by learned brāhmaṇas, I wish, in exchange, that this brāhmaṇa be freed from the burning caused by the Sudarśana cakra.

SB 9.5.11: If the Supreme Personality of Godhead, who is one without a second, who is the reservoir of all transcendental qualities, and who is the life and soul of all living entities, is pleased with us, we wish that this brāhmaṇa, Durvāsā Muni, be freed from the pain of being burned.

SB 9.5.12: Śukadeva Gosvāmī continued: When the King offered prayers to the Sudarśana cakra and Lord Viṣṇu, because of his prayers the Sudarśana cakra became peaceful and stopped burning the brāhmaṇa known as Durvāsā Muni.

SB 9.5.13: Durvāsā Muni, the greatly powerful mystic, was indeed satisfied when freed from the fire of the Sudarśana cakra. Thus he praised the qualities of Mahārāja Ambarīṣa and offered him the highest benedictions.

translation done by His Grace Jambavan Prabhu : I offer my respectful obeisances to the Supreme Lord. O Sudarçana cakra, please destroy the poverty and distress of those who have not committed offenses. Please kill the sinful. Please give the sinful terrible diseases. I offer my respectful obeisances to You, the thousand spoked one, for You instill fear in the three worlds. O Lord Janärdana, please quickly order the destruction of all miseries and demons. I offer my respectful obeisances to the most eminent Sudarçana cakra, who is most fierce and terrifying. O great of , O most grave one, O most powerful one, please devour all our enemies. Please consume all the powerful mantras along with all the powerful energies. Please consume the foolish as well as their servants. Consume and devour the Daityas, Dänavas, Räkñasasa, Däkin , Säkinëés, Vetälas and others. Please also consume the Gandharvas, Yaksas, ghosts, evil spirits, Pisäcas, and the Brahma-räksasas.
I pray that anyone who reads this article will have their obstacels on the path of Bhakti removed
